Refunds

The information required below will determine if we can process you return / refund application. Please note that any refund or return that is made MUST be in line with our Returns / refund Policy.

All information supplied must relate to the actual order placed and the application must be made by the person that made the initial booking / purchase.

Any refunds agreed will be credited to the original card used for Payment.


    If a home use kit please confirm the return details

    Please note that you will be informed by email of the outcome of your refund request. This will be sent to the email address you have declared in this form. Once a decision has been made, there is no right to appeal so please ensure you provide all necessary evidence and information to support your refund request.

    All refunds that are agreed will be in line with our Stated Terms and Conditions and refund / returns policy - copy can be found here and any refund may be subject to the stated Admin fee per test.
